Intermediate Care Criteria

Intermediate Care is a name given to a range of services, on discharge from hospital, that some people are eligible for.

These services will differ from county to county. In Swindon’s case, these services are designed or commissioned by NHS Swindon and Swindon Borough Council. 

Intermediate Care requires that you agree to, and can fully participate in, a structured, individual care plan that involves active therapy, treatment or an opportunity for recovery.

This must be a planned outcome of maximising independence, typically enabling you to resume living at home. You will not be charged for the service provided by the Community Intermediate Care Team.

The length of time that you will be receiving community intermediate care services for, will be tailored to your progress, and therefore may vary from a few days up to six weeks.
